YES, I would like to participate as a judge for the 2012 National Health Law Moot Court Competition. Please check the time(s) that you are available. We will schedule you for no more than 2 rounds.
NO, I am unable to participate as a judge in this year's Competition, but please contact me again next year.
If you checked the “YES” box, please indicate your availability below. Please check as many as you’d like; we’d like judges to stay for two rounds if possible and will, at most, schedule you for two rounds unless you otherwise indicate.

Comments:
My travel time to SIU exceeds two and 1/2 hours and, given the time(s) of the round(s) I'm judging, I will need a hotel room which will be reserved and paid for by the Competition.
We have reserved double and king rooms for the judges. An email room reservation confirmation will be emailed to you prior to the Competition.
